Plans and implements food service or nutrition programs throughout the Hospital to help promote health and disease control
Conduct initial nutritional assessments of inpatients to determine the nutritional status or nutritional education needs of new patients
Conduct regular nutritional assessments based on the patient's condition, treatment plan, individual needs, or if there are significant changes in the patient's condition to determine the patient's response, follow-up care plan and/or discharge plan
Analyze integrated information and patient assessment data to prioritize patient needs
Monitor the implementation of the patient's diet therapy plan and/or daily food intake to ensure that the nutritional services received are in accordance with the predetermined plan of care
Providing education and nutritional/dietary counseling to inpatients, outpatients, MCU and HD to ensure a smooth healing/treatment process
Collaborate with catering services in developing standard recipes and hospital food menus to ensure consistency and satisfaction with the quality of food provided to patients and workers
Develop a liquid food formula to be used as a standard for providing food for patients who use feeding tubes
Conduct audits of procurement, receipt, preparation, processing/production, storage, distribution and cleanliness of food ingredients to ensure that food safety standards are achieved in accordance with applicable laws/regulations
Carry out tests and check patient food to ensure the quality of the food served and the patient's nutritional intake is in accordance with the patient's needs and condition
Controlling assets and ensuring the provision, replacement and maintenance of space, technology, equipment and other resources in accordance with the needs and conditions of the Hospital
Carry out complete documentation before, during and after treatment to be used as a reference for disease history and patient visit statistics
Collect required data and/or make reports related to operational activities as accountability to superiors
Implement procedures and practices for quality and safety services, including attending training, collecting data, documenting and/or making reports needed to support the achievement of hospital wide indicators and related department/unit targets
Qualifications
Diploma (D3) Degree from Nutrition
Have minimum 1 year work experience in nutrition services
Clinical experience in nutrition, preferably within a hospital or healthcare setting.
In-depth knowledge of clinical nutrition, including dietary management for various health conditions.
Strong skills in assessing nutritional needs, developing care plans, and monitoring patient progress
